The SGH Medical Social Services Community Care Team (MSS CCT) was established in 2018 with the mission of serving as an interface between the hospital and the community. Its objectives are to shape the health-social care landscape through leadership and advocacy, and to strengthen and establish partnerships to meet the diverse needs of our communities.
We envision a future where transitions in care are seamless, communities are vibrant and empowered, and holistic health and social care become the norm. Our medical social workers are dedicated to improving access to biopsychosocial care and supporting population health initiatives. We incorporate a preventive health lens, promote health-seeking behaviors, and demonstrate agility in our care delivery.
MSS CCT collaborates with health and social care partners to bridge care from our hospital to the community. We co-create interventions to meet residents’ needs through asset-based community development approaches and strive to improve health equity. In addition to clinical services, the team conducts community outreach and supports capacity building through training and education.
